The Referral Process
In order to ensure that appropriate goals and objectives are set and agreed by all concerned, the Stroke and PAT project works on a referral basis. The referral form can be completed by any of the people in the following list, who may be involved in your care:
- General Practitioner
- Stroke Navigator
- Occupational Therapist
- Speech and Language Therapist
- Psychologist / Counsellor
The referral form can be downloaded from the menu bar above, completed and then sent via our secure email to: email@example.com. Alternatively, the form can be printed off and then sent via the post to: Sallie Bollans, Stroke Rehab Dogs Director, care of the Ruth Winston Centre.
Upon receipt of the referral your referrer will be contacted to either accept or decline your request. If declined, an explanation as to the reason why will be given. It is important to note that Animal Assisted Therapy is not always an appropriate choice of intervention and the welfare of the client and the animal will always be equally considered for every referral received.
If you are successful in your application, you will be invited to come along and observe the end of a session in action prior to beginning your course of treatment. This gives an opportunity for you to be sure that this type of intervention is for you.
All confidential information will be communicated and stored in line with the Data Protection Act 1988 and in accordance with the Freedom of Information Act 2000.
